The Case Manager/Support Worker will provide case management for assessment, treatment and assertive signposting to existing services outside of the hospital setting for a time limited period. As such, the role will require active management of administration and contribution to the coordination of the MDT and services, as well as engagement with a wide range of stakeholders internally and externally to UCLH.

Therefore, the ability to work in an innovative and in a flexible manner to support with the delivery of the service, with support from senior managers, is a key requirement

Main duties of the job

The role requires the use of specialist knowledge, good relationship skills as well as considerable resilience to empower service users who may have varying degrees of mental health, addiction and trauma related issues to engage with health and social care services.

This will include having an advocacy role on behalf of the service user to access mainstream services, accompanying the individual to appointments and assisting them with housing, financial and health and wellbeing needs. The post-holder will be a core member of the Integrated Operations Service working on the delivery and operational output of the HIC.

The postholder will demonstrate the ability to work effectively within a multidisciplinary led service, as part of the clinical and operational team in the delivery of a wraparound model of care for homeless people within NCL.

Detailed job description and main responsibilities

Working within the Integrated Operations service you will be joining the team at a very exciting time with the ongoing development of this successful and established service called the Homeless Intermediate Care Team (HICT), which forms part of the NCL Out of Hospital Care Model (OOHCM). The NCL OOCHM is one of seventeen pilot sites across England funded by the Department of Health and Social Care (DHSC).

The OOHCM supports people experiencing homelessness with health and social care support and temporary accommodation after being discharged from hospital. The vision is to end the cycle of the homeless population leaving hospital often being discharged back to the streets who then quickly re-present to emergency departments.

The Homeless Intermediate Care Team (HICT) is aligned to Discharge to Assess (D2A) principals focusing on patients being discharged from UCLH, Royal Free and Whittington hospitals and their respective D2A P2 stepdown beds. The service works with adults who are experiencing homelessness, rough sleepers and those at risk of rough sleeping including those with no recourse to public funds (NRPF).

Knowledge and Qualifications

Essential criteria

  • NVQ Level 3 or equivalent relevant experience / HSC Diploma Level 3 Advanced / Higher apprenticeship Level 3

Experience

Essential criteria

  • Experience in carrying out assigned duties after training
  • Working in a team
  • Working in a Health Care or related setting
  • Good standard of written and spoken English
  • Experience of working with inclusion health population e.g. those experiencing homelessness
