Qualifications
Essential criteria
- First Level NMC Registration
- Recognised teaching assessment qualification/accreditation
- Diploma in Health related subject
- Evidence of recent CPD
- Evidence of working with patients with Long Term Conditions
Desirable criteria
- Documented evidence of having undertaken relevant study in relation to Long Term Conditions
- First Degree or working towards
- Leadership qualification
- Community experience
Experience
Essential criteria
- Significant post registration experience
- Demonstrate a working knowledge of community nursing, case management and management of a patient within a community setting
Desirable criteria
- Experience of working within other relevant areas of healthcare
Skills/Knowledge
Essential criteria
- Excellent communication skills (written & verbal)
- Effective time management
- Assertive
- Change manager
- Good Health and Social Assessment Skills
- Knowledge of current Health and Social Care Legislation affecting people with Long Term Conditions
