COVID-19 vaccination remains the best way to protect yourself, your family, your colleagues and of course patients from the virus when working in our healthcare settings. Whilst COVID-19 vaccination is not currently a condition of employment, we do encourage our staff to get vaccinated. We will be checking the vaccination status of all new starters so that we can manage individual and environmental risks, and so that we can support those who may be undecided about vaccination.

If you are unvaccinated, there is helpful advice and information available by searching on the 'NHS England' website where you can also find out more about how to access the vaccination. There is currently Government consultation underway which will determine whether some new employees may need to be vaccinated.

Qualifications

Essential criteria

• Registration with NMC
• Recognised teaching/mentor qualification

Desirable criteria

• Non medical prescriber V300
• Documented evidence of having undertaken relevant further study or training appropriate to the role of Case Manager
• Degree/diploma in Clinical Nursing or evidence of working towards
• Leadership/management qualification

Experience

Essential criteria

• Significant post registration experience
• Demonstrate a working knowledge of community nursing, case management and management of a patient within a community setting

Desirable criteria

• Experience of working within other relevant areas of healthcare

Skills/Knowledge

Essential criteria

• Excellent communication skills (written & verbal)
• Effective time management
• Assertive
• Change manager
• Good Health and Social Assessment Skills
• Knowledge of current Health and Social Care Legislation affecting people with Long Term Conditions
• Teaching Skills for patients, carers and staff.
• Ability to manage Long Term Condition patients
• Ability to develop and sustain partnership working within individuals and across organisations
• Educator, mentor/supervisor, facilitator, role model

Desirable criteria

• Evidence of management development or courses
• Presentation skills

Other

• Abilty to travel between bases within the community
