THE PURPOSE OF THE ROLE
As Case Manager Assistant you will be responsible for providing day to day operational support to a team of Clinical Case Managers.
WHAT YOU WILL BE DOING AS CASE MANAGER ASSISTANT
- Undertake research as directed for the sourcing of services, equipment and other client related requirements.
- Initiate and coordinate the management of the third-party provider process.
- Undertake the administration and recording in relation to client funding requests for the above.
- Provide monitoring support to the client account process ensuring case managers have access to accurate balance information when needed.
- Assist clients to prepare for meetings and appointments.
- Liaise with members of the multi-disciplinary team as required.
- Follow up request for GP and medical records.
- Undertake all required administration requirements as indicated by the identified case managers.
- Be responsible for the organisation of client related meetings, ensuring invitations are issued, agendas produced, accurate minutes maintained and circulated.
- Be responsible for the gathering of pre meeting reports and summaries as required.
- Undertake direct client related work as allocated by the case manager. This will include the establishing of timetables, implementation of agreed strategies and monitoring to report back on their effectiveness
- Set up taxi accounts
- Ensure contacts are added correctly.
- Booking taxis for clients.
- Flagging up when goal sheets and risk assessments need to be updated / reviewed.
SKILLS AND EXPERIENCE REQUIRED
- Excellent written and verbal communication skills.
- Microsoft Office proficient.
- Experience working in a clinical setting with individuals with disability or who are vulnerable.
- Excellent organisational and interpersonal skills.
- Ability to work independently on a broad variety of projects with competing deadlines.
