Our stunning care homes Grappenhall Manor Centre and Statham Manor Care Centre are located 4 miles apart in the Cheshire area. The homes are registered to provide care to 70 and 66 residents respectively, with a variety of care needs including residential, dementia and nursing. Each of these beautiful homes also provide intermediate care in association with Warrington & Halton NHS Trust to provide rehabilitation following hospital admissions. The teams at Grappenhall and Statham Manor work closely with the NHS therapy team to reach rewarding outcomes for the residents in their care.

We are seeking to appoint an experienced and motivated Bed Manager to join our team. Experience in a clinical environment is essential to apply for this role. As the Bed Manager you will report directly to the Home Managers at both Grappenhall and Statham. You will work closely with Warrington & Halton NHS Trust in order to provide a comprehensive patient flow service to support the clinical activity of the Discharge to Assess communities across both care homes. We are offering an excellent salary of £26,500 per annum. This is a full time contract with working hours of Monday - Friday, 0900 - 1700.

Role Responsibilities:

  • Review initial assessments regarding prospective temporary residents' needs
  • Coordinate admissions from Hospital to discharge to assess/intermediate care beds
  • Communicate effectively with a wide spectrum of multi-disciplinary professionals to achieve the best outcomes
  • Coordinate quality and safe discharges in a timely manner
  • Support the care homes, and external discharge teams with residents who require additional, individual planning
  • Support MDT meetings both internally and externally, providing updates and information
  • Attending daily bed meetings with external stakeholders providing up-to-date bed availability, pending discharges and admissions due
  • Provide a person-centred, quality and effective service

Skills and experience required:

  • Previous experience in a clinical environment with knowledge/experience of continued care planning and delivery desirable
  • Confident in liaising with management and external professionals across multiple disciplinaries and able to maintain effective professional relationships
  • Good time management and organisation skills
  • Excellent communicator
  • Able to work using own initiative, contributing to the wider team in order to achieve the best results
  • Confident using IT systems to maintain up-to-date records
  • Minimum requirement of NVQ Level 3 in Health & Social Care unless expansive experience can be demonstrated
