Job overview

Social prescribing enables health and care professionals to refer people to a range of local, non-clinical services to support their health and wellbeing. Transformation Partners in Health and Care have played a pivotal role in the development of policy, strategy, innovation, and operational delivery of Social Prescribing across London over the last 5 years. We support London's health and care systems to scale community centred models of care which focus on addressing the wider social determinants of health and target those experiencing the greatest health inequalities

As a Programme Manager you will be overseeing a programme of work supporting the development of Social Prescribing and Community Based Prevention across the capital. We are looking for someone who is passionate about how London's health and care system can scale community centred approaches which focus on addressing the wider social determinants of health and target those experiencing the poorest outcomes. Having a broader understanding of community development, population health, and service design is desirable. We are looking for someone with experience of developing collaborative partnerships with a breadth of stakeholders and has a solid understanding of navigating the complexities of system wide transformation.

Main duties of the job

Important aspects of this role include:

  • Overseeing development & delivery of projects across the programme
  • Supporting the team to deliver its priorities including line management of project managers in the team
  • Developing partnerships, stakeholder engagement & management

  • Masters degree level education or equivalent experience
  • Further training and experience in project management and/or performance management and/or business process activity and/or experience of NHS change management processes
