An exciting opportunity has arisen for a:
Band 7 - Senior Commissioning Practitioner, to work in the All Age Continuing Care Service in the NHS Cheshire and Merseyside ICB, (Cheshire East based).
Main duties of the job
Do you have the skills and experience to make a difference to service users, then this post may be for you. We are looking to recruit a Band 7 - Commissioning Practitioner within our All Age Continuing Care Service.
The post will provide specialist nursing assessment, procurement, case management and review for adults and young people in transition to adult services presenting with complex long-term care needs, who are eligible or potentially eligible for NHS Continuing Health Care or Funded Nursing Care.

Job responsibilities
Successful candidates will be able:
- Provide assurance around quality of commissioned care packages, reporting safeguarding concerns in accordance with agreed interagency reporting procedures.
- Carry a case load of the most complex client group, including adults who have experienced an acquired brain injury and /or require medical ventilation or tracheostomy care.
- Ensure safe discharge and transfer of individuals from highly supportive and acute care environments to community placements avoiding out of area placements. To ensure timely and robust ongoing reviews are completed for individuals in caseload including preparation for court of protection and review of deprivation of liberty safeguards.
- Deliver against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines.

- Degree or equivalent level of experience in health or social care.
- Current NMC Registration.
- 2 year experience of managing complex cases.
- Management / leadership qualification or experience.
- Evidence of professional development.
Desirable
- Relevant post basic academic / professional qualification at higher degree level or equivalent
Knowledge
Essential
- A clear understanding of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing care.
- A clear understanding of The National Framework for children's and young people's continuing care.
- Demonstrable relative experience and knowledge of current key policies and priorities.
- Understanding of service provision for the client group.
- Understanding of deprivation of liberty safeguarding processes.
- Sound knowledge and understanding of the current NHS reforms.
- Understanding of the personalisation agenda.
