Assistant Team Manager - Adults Intermediate Care Team

Salary: Band 11- £44,428 - £48,474p.a.

37 hours per week

The Service

There have been exciting changes within Portsmouth Adults Intermediate Care Team which means we have an Assistant Team Manager vacancy to fill.. The new service went live on 1st April 2024 and has brought together a number of teams to provide a single approach to supporting people who are moving through admission avoidance, the acute hospital, inpatient settings, and rehabilitation and reablement services. This is an exciting time to join us and help us on our journey to improving outcomes for people following a crisis or decline in health.

This post is based within the Portsmouth Adults Intermediate Care Team supporting our integrated services including, Community nursing, Urgent Community Response, Portsmouth Rehabilitation and Reablement team, The Transfer of Care Hub, the acute Hospital and Inpatient units. We deliver a high-quality service and those who use the service are at the heart of everything we do. What is the role

We are looking for an experienced Assistant Team Manager who can support us on our improvement journey. You will need to be able to operate with a high level of autonomy and authority using a wide range of evidence-based social work interventions and tools. You will provide a high degree of professional support to staff, leading on practice development to improve performance and quality of service delivery and ensure integrated, multi-agency working is at the heart of improving outcomes for adults with care and support needs. You will consistently demonstrate a high level of competence and confidence in the full range of social work tasks, particularly in relation to Adult Safeguarding, adults with complex care and support needs, mental capacity assessments, DoLS and partnership working.

You will be supporting the team manager and line managing a group of social workers and other professionals/practitioners, equipping them to make effective decisions and to be professionally resilient, enabling them to ensure safe and effective outcomes for our residents. This will include undertaking supervision, prioritising resources, monitoring and evaluating service delivery, disseminating best practice, managing individual performance, report writing and supporting practitioners to represent PCC in court when required.

What you'll need to succeed

You will need to be a Social Worker registered with Social Work England and be passionate about putting people at the heart of what you do, understanding the needs of the client group as well as promoting, and supporting individuals' rights and choices. You will be able to support, educate, and develop practitioners to their full potential but taking the necessary steps to address performance issues when required. Be fully conversant with the Care Act, the Mental Capacity Act, Adult Safeguarding legislation, and other applicable legislation. You will have excellent interpersonal skills and be able to work in collaboration with and communicate effectively with service users, staff, peers, professionals, and more senior leaders to ensure we deliver a robust, accountable offer to those that access our service.
