About The Role

We have an exciting opportunity to join the Home First team as an Area Practice Manager supporting discharges from hospital for Dorset residents.

This opportunity is available to individuals either working currently in an APM role and are looking to widen their service experience or practitioners considering a management role and is open to both Social Workers and Occupational Therapists.

Home First is a fast-paced environment where no two days are the same! As part of a Dorset system response, we work as part of the integrated care system delivering a discharge to assess model which will see most assessments taking place outside of hospital to determine an individual's ongoing support. As an Area Practice Manager, you will work alongside our existing APM's supporting our staff to deliver care act assessments to support discharge; currently assessments take place both in and outside of hospital depending on an individual's journey. APM's are aligned to cluster areas North, East and West, with direct line management of both Social Work and Assessment Support Co-ordinators.

We are looking for candidates with a strong strength-based approach who can lead, motivate, and develop teams. This is an exciting time to lead within Adult Social Care as you will be involved in transforming the way we work and have an integral role in positively leading and implementing change to meet both organisational and customer demand.

This APM role is covering the East Cluster, mainly Bournemouth and Poole Hospitals.

The team will be working together two days a week at the Ferndown Office.

This post requires significant travel. You must have a vehicle (or transport we deem suitable) available for use as needed.
