About the role

An exciting opportunity has arisen for a Senior Social Worker to join us at Cambridgeshire County Council's Continuing Healthcare team, on a permanent full-time basis.

Working across Cambridgeshire and County wide you will work in partnership with the Integrated care Board to identify and determine individual's eligibility for NHS Continuing Healthcare through joint completion of the NHS Continuing Healthcare process alongside social worker statutory responsibilities.

What will you be doing?

The role involves supervising and supporting a small team of colleagues with attending Continuing Health Care Decision Support Tool meetings. As a Senior Social Worker you will also be supporting with the completion of Care Act 2014 Assessments, or Reviews, Mental Capacity Act 2005 Assessments and Best interest Decisions (where applicable) evidencing the Local authority's contribution towards the NHS Continuing Healthcare process and supporting the Multidisciplinary teams' recommendations on eligibility for NHS Continuing Healthcare. Where an individual is not eligible for NHS Continuing Healthcare but is eligible for care and support under the Care Act 2014, the successful candidate will support team colleagues with implementing care and support. Senior Social Workers will also be expected to hold their own small complex case load including the attendance of Decision Support Tool meetings and report to the Team Manager.

You will also support the Adult Social Care workforce across Cambridgeshire to improve their confidence, skills, and knowledge in relation to NHS Continuing Healthcare. This involves providing, training, coaching, mentoring and advice and guidance to staff in relation to NHS Continuing Healthcare.

In addition to the responsibilities outlined above and in the Job Description, the post holds the following additional responsibilities:

  • Support the Adult Social Care Work force by providing advice, and guidance in relation to the National Framework for NHS Continuing Healthcare
  • Assist in the delivery of training across the County in relation to NHS Continuing Healthcare.
  • To provide critical analysis of information and write reports to Court standard
  • Provide support in dispute meetings.

About you

You will be passionate about individual's rights and NHS Continuing Healthcare, a qualified Social Worker registered with Social Work England, hold a full driver's licence and be willing to travel for work, willing to learn and be flexible in your approach, experienced in NHS Continuing Healthcare, and experienced or interested and capable in supervising or supporting colleagues.

You must:

  • Be a qualified Social Worker with a Social Work England registration
  • Have the ability to work from a variety of locations and travel countywide independently
