Role Summary

We are seeking to recruit a dynamic, enthusiastic and hardworking individual to join us as a Band 5 Administrative Line Manager who is responsible for the line management of administrative and clerical staff and the day to day running of administrative functions, within the Hospital Discharge Team.

This person must have proven skills and experience in relation to discharge planning from hospital. They will be used to working with all members of the multidisciplinary team within both the acute and community settings, the wider health and social care teams and other private care providers such as domiciliary care providers.

Excellent communication skills are essential as the role will involve liaising with all of the above as well as patients and their advocates, often in difficult circumstances.

An understanding of the discharge pathways from hospital and how delays are identified, recorded and addressed would be an advantage.

Advanced IT skills are required and the ability to use relevant health and social care electronic systems also an advantage.

Working Pattern: The post holder will be required to work full time hours, with the shift pattern covering Monday to Sunday 0800 to 1800 hours, currently.
