CLCH Hertfordshire Adult Community Services - CLCH NHS Trust provides a range of Community Services, and the Hertfordshire Division is recruiting!

Hi! My name is Louise and I am the Head of Clinical Services for the Planned Care Clinical Service Unit in the CLCH Hertfordshire Division. I have been in this role for 5 years and can proudly say that the Planned Care service is made up of an amazingly committed, talented, creative, and adaptable group of staff who have achieved some remarkable things in exceptionally difficult times. Following internal promotion of the post holder we are looking for a new Locality Manager for our Dacorum Integrated Community Team.

The Planned Care Clinical Service Unit consists of Community Nursing, Community Physiotherapy and Occupational Therapy, Domiciliary Phlebotomy, Complex Case Management and Falls and Frailty services. This role will be to head up the Dacorum Locality Integrated team and the West Herts Overnight Nursing Service.

Main duties of the job

Applicants will be familiar with community services and have operational management experience at Band 7 level, a track record of successful delivery of change management and service improvement projects, and a current registration with the NMC or HCPC.
