  • To assess for, plan and implement therapy interventions and treatment plans, such as exercise programmes (group or individual), provision of mobility aids, measuring and fitting adaptive equipment, minor and major adaptations.
  • Ensure equipment is used safely, following relevant training, and that it is maintained appropriately.
  • Use your skills for making ongoing evaluations of patient's response to prescribed treatment/care plans, and prepare for supervised patient discharge.
  • To effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communication, e.g. hearing loss, diminished sight, cognitive impairment, behavioural problems, and pain.
  • Establish and maintain appropriate working relationships with patients, carers, colleagues, and other health care professionals. This will also enable you to assess patient understanding of treatment proposals, gain valid informed consent and to work within a legal framework with patients who lack capacity to consent to treatment.
  • To identify and initiate onward referral to other services for patients as appropriate.
  • Staff development within CPFT is highly valued and you will be supported to attend relevant internal and external courses to develop your knowledge and skills to improve clinical practice as identified as part of your Appraisal process.

Education/Qualification

Essential criteria

  • NVQ Level 3 in Health and Social Care or equivalent level of knowledge and experience
  • Good general education up to GCSE equivalent including Maths and English
  • Willingness to undertake training to meet competency framework

Desirable criteria

  • NVQ Level 3 Care Award (certificate & portfolio)
  • Chair based and OTAGO exercise training, or willingness to undertake

Experience

Essential criteria

  • Clinical experience in a rehabilitation/reablement or therapy environment

Desirable criteria

  • Community experience
  • Knowledge of falls prevention

Skills & Abilities

Essential criteria

  • Able to demonstrate commitment to high quality care and service provision
  • Ability to work flexibly as part of a team
  • Good communication skills

Physical Requirements

  • Kneel and bend and work in cramped environments
