In the community, we deliver services that focus on people with long term conditions living well outside hospital, through our integrated locality teams working with district and community nursing teams. Community members of staff also provide care to patients in their homes and from a number of clinics, health centres and GP surgeries.
Job overview
The Core services Division at East Sussex Healthcare NHS Trust have an exciting opportunity for a Macmillan information and support manager to join our personalised care team. This role will be vital to support the design and development of a high quality, supportive and inclusive local information and support service within East Sussex.
As part of the Core services division, the personalised care team comprises of the Cancer personalised care lead nurse and Macmillan lead cancer counsellor with close links to the Lead Cancer nurse, and other Cancer teams within ESHT.
We are looking for a highly motivated individual with a passion for delivering high quality patient support to patients and the public with a specific focus on Cancer. The successful candidate will demonstrate knowledge and experience of working with patients with cancer and their loved ones. The successful candidate will be passionate about providing and delivering information and support with strong links to the community and voluntary sector
Main duties of the job
The main duties of this role are to:
• To support promotion and development of Macmillan information and support hubs within ESHT and at satellite sights.
• To increase patient, carers and health care professional engagement throughout the cancer pathway
- To ensure more people have access, from diagnosis, to a good conversation about their needs and are given help to navigate and access sources of support.
• Provide Macmillan information and support service (MCIS) information outside the Trust as an outreach cancer service within the East Sussex integrated care board, to ensure that people living with cancer and hard-to-reach communities are supported outside of the hospital setting.
• Engage with key stakeholders to develop innovative Information and Support services in acute and community settings within East Sussex ICB.
• Enable networking and collaboration of information providers supporting people affected by cancer across East Sussex Healthcare localities to help develop a seamless 'Macmillan experience' for people with cancer within East Sussex ICB.
• Develop new partnerships which support the sustainable service delivery of the project through appropriate local community partners.

Essential criteria
- Good general level of education including Maths and English
- Knowledge base of cancer
- Relevant post-graduate specialist course
- Evidence of ongoing professional development to Masters level or equivalent level of knowledge and experience
- Recognised leadership development training
